Job Description:
  • Undertake assessments under the Community Care Act and other legislation, using a self-directed support approach.
  • Coordinate and monitor care packages and support (care) plans, and undertake reviews to ensure desired outcomes are achieved. This may include advocating on behalf of service users or carers.
  • Work with individuals to assess risk and make decisions related to their ability to function in their community environment.
  • Lead Safeguarding Adults investigations using the local ‘No Secrets’ multi-agency policy and procedure.
  • Promote safeguarding awareness with service users and multi-disciplinary colleagues. Keep updated on current safeguarding guidance and practice, and be vigilant in all activities.
  • Handle complex cases involving safeguarding issues, family dynamics, conflict, and problematic situations.
  • Provide written reports for court proceedings, case conferences, and other forums.
  • Work with highly complex risks, especially where there are capacity or best interest issues.
  • Engage in inter-disciplinary relationships and partnership working, encouraging integrated approaches where appropriate.
  • Operate according to NCC and departmental practice, including maintaining electronic social care records within CareFirst and following relevant procedures.
  • Provide practice guidance to less experienced staff under supervision.
  • Supervise competency-based staff and students, ensuring compliance with record-keeping and data quality standards.
  • Ensure services are culturally sensitive and advocate for access to departmental and health resources.
  • Develop partnerships and resources to benefit service users.
  • Maintain good relationships with partner agencies and promote a positive departmental image.
  • Familiarize partners with departmental tasks and responsibilities.
  • Participate in inter-agency training and staff development aligned with CPD strategies.
  • Undertake training to qualify as a Practice Educator, AMHP, or BIA as appropriate.
  • Provide services in accordance with departmental standards, ensuring equal access, diversity, and safety, while being cost-effective.
