Key Responsibilities

* Deliver a high-quality field social work service , focusing on promoting independence and wellbeing.

* Support adults with a range of needs, including physical disabilities, learning disabilities, sensory impairments , and life-limiting conditions .

* Undertake Care Act assessments , develop support plans, and review care packages.

* Commission and monitor services, ensuring they meet identified needs while working within budgetary constraints .

* Maintain accurate and timely case notes in line with local authority policies and relevant legislation .

* Collaborate effectively with families, carers, health professionals, and community organisations.

Ideal Candidate Profile

* Qualified Social Worker with current Social Work England registration .

* Strong knowledge of the Care Act 2014 , Mental Capacity Act 2005 , and safeguarding procedures .

* Experience working in locality or community-based teams is preferred.

* Excellent assessment, communication, and case management skills.

* Ability to balance autonomy and teamwork in a fast-paced environment.
