Requirements:

  1. Care Coordination: Ensuring all aspects of a patient’s care are organized and communicated across providers.
  2. Health Assessments: Regular evaluations of the patient’s health status.

  4. Support Services: Access to social services, mental health support, and community resources.

What we are looking for in a Social Worker:

  1. Social Work England registration
  2. Eligible to work in the UK
  3. Hold a full UK licence
  4. Post-qualified experience
  5. Degree level or equivalent in Social Work
  6. Twice weekly payroll
