This is an exciting opportunity for a Social Worker within the Leaving Care Team in Swindon! What you will do

• Manage a caseload of young people preparing to leave care and transition to independence.
• Work alongside Pathway Advisors to develop, complete, and maintain pathway plan assessments and initial pathway plans.
• Ensure compliance with required legislation and maintain plans until the young person turns 18.
• Participate in the duty system, providing additional support to young people when necessary.
• Collaborate with other services to ensure seamless support for young people during their transition.
