Responsibilities
  • Improve outcomes for adults with care and support needs, as well as carers, by completing strengths-based assessments.
  • Develop and implement effective care plans working in accordance with the Care Act 2014.
  • Undertake safeguarding duties as required to protect adults with care and support needs from abuse or neglect.
  • Practice social work in a transparent, accountable, and safe manner, adhering to professional standards and ethics.
  • Liaise with other professionals and agencies to coordinate an appropriate level of intervention and care.
  • Monitor and review care plans and interventions as needed to maximise outcomes.
