Job summary

Liquid Personnel is recruiting a Social Worker to join the Learning Disabilities Review Project Team in Cambridgeshire. You will review complex and high-cost care packages for adults with LD and Autism, using strengths-based, person-centred practice to improve outcomes and maximise wellbeing.

Qualifications

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care.
  • LD & Autism Experience: Proven experience working with adults with Learning Disabilities and Autism.
  • Legislation Knowledge: Strong understanding of the Care Act 2014 and Mental Capacity Act 2005.
  • Assessment Skills: Experience undertaking complex assessments, reviews, and evaluations of care and support arrangements.
  • Care Package Reviews: Experience reviewing complex and high-cost care packages and making evidence-based recommendations.
  • Multi-Agency Working: Strong experience working collaboratively with brokerage teams, contracts teams, providers, carers, advocates, and partner agencies.
  • Communication Skills: Excellent assessment, analytical, report-writing, and professional communication skills.
  • Driving Licence: Access to your own vehicle is essential.

Responsibilities

  • Completing Care Act 2014 assessments for adults with Learning Disabilities and Autism, incl. complex/high-cost packages.
  • Undertaking timely reviews following changes in needs, circumstances, or support arrangements.
  • Reviewing care and support packages to ensure proportionate, effective, outcome-focused, value for money.
  • Delivering a minimum of two completed and recorded assessments per week.
  • Conducting face-to-face assessments and reviews with individuals and their support networks.
  • Collaborating with advocates, carers, families, providers, and partner agencies for a person-centred approach.
  • Analysing and evaluating care and support arrangements to identify strengths, risks, gaps, and opportunities.
  • Collaborating with Contracts and Brokerage Teams to develop cost-effective and appropriate solutions.
  • Promoting independence, wellbeing, choice, and control through strengths-based planning.
  • Undertaking Mental Capacity Assessments and contributing to Best Interest decisions.
  • Maintaining accurate, defensible, and timely records, assessments, reviews, and professional reports.

Liquid Personnel is recruiting Social Worker for its client's Learning Disabilities Review Project Team at Cambridgeshire.

This is an excellent opportunity for a skilled practitioner to work within a county-wide specialist team supporting adults with Learning Disabilities and Autism (LD&A). The role focuses on reviewing complex and high-cost care packages to ensure they continue to meet individual needs, deliver positive outcomes, promote independence, and provide value for public resources.

Working closely with Senior Social Workers, Contracts Teams, Brokerage Services, individuals, families, carers, and advocacy services, you will undertake comprehensive reviews and assessments of existing support arrangements. The successful candidate will be experienced in strengths-based and person-centred practice, with the ability to analyse complex situations and make evidence-based recommendations that improve outcomes and maximise wellbeing.

What will your responsibilities be?
