Location: Cheshire West (Hybrid)

Pay Rate: £35 per hour

About the Role:

Cheshire West Council is seeking a dedicated Social Worker to join their Learning Disabilities Team. This role offers the opportunity to impact individuals with learning disabilities by providing tailored support and advocacy.

Responsibilities:

• Assess the needs of adults with learning disabilities and develop tailored support plans.
• Build effective relationships with service users, families, and multidisciplinary teams.
• Conduct mental capacity assessments and ensure adherence to the Mental Capacity Act.
• Safeguard vulnerable adults by identifying risks and taking appropriate actions.
• Collaborate with health, education, and housing services to facilitate care packages.
• Maintain accurate records and ensure compliance with policies and statutory requirements.
• Engage in reflective supervision and professional development to uphold high standards.
