Connect2Hampshire is seeking compassionate Social Workers to join the Learning Disabilities team on a 6 month contract in Totton. You will handle a varied caseload, perform risk assessments, and work within a multi-disciplinary team to safeguard and support service users.

Role requires experience in learning disabilities, collaboration with health, social care and education partners, and ability to conduct home visits with hybrid working options. 37 hours per week; umbrella rate £38 per hour.

Qualificações

• Qualified social worker with registration and registration status confirmed.
• Experience in learning disabilities or similar multidisciplinary teams.
• Ability to work across agencies including health and education.

Responsabilidades

• Make ongoing caseload assessments reflecting risk, independence and need.
• Respond to referrals with appropriate action and liaison with teams.
• Apply community care and risk assessment in safeguarding individuals.
• Complete statutory reviews and ensure policy compliance.
• Assess needs with clients and professionals to agree support levels.
• Engage with service users who are reluctant to accept services.
• Negotiate sensitively where there is conflict or lack of insight.
• Manage crises using relevant legislation and resources.
• Assess carers' needs and develop carer support plans.

We are seeking compassionate and dedicated Social Workers to join our learning Disabilities team on a 6 month contract.

Location: Totton

Main Responsibilities and Duties

• Make ongoing assessments of allocated cases to reflect individual circumstances including assessment of risk, enabling independence and need to ensure that service users are protected from significant harm.
• Responsible for a caseload of varying complexity, commensurate with ability and qualifications, receiving and responding to referrals from various sources.
• Community Care, integrated care programme approach and risk assessment to work to protect the safety and protection of individuals.
• Statutory reviews and comply with appropriate legislation, policy and procedures.
• Assess client needs in consultation with them and with other professionals and agree levels of support.
• The need to engage with people refusing a service who are at risk to self or others.
• The need for sensitive negotiation when assessing service users and carers where there is conflict or lack of insight.
• Effectively manage people presenting in crisis needing a quick resolution, using relevant legislation and resources.
• Statutory responsibility for the assessment of carers (including young carers) needs and the formulation of a cars care plan.

Requirements

• Previous experience in a Learning Disabilities team.
• Relevant multi-disciplinary Social Care experience within multi-disciplinary team relevant to needs of post
• Evidence of partnership working with other agencies including Social Care Services, Health Services, Voluntary Agencies and/or Education
• A thorough understanding of relevant legislation, policies, and best practices in social work.
• Ability to work independently as well as part of a multidisciplinary team.
