Social Worker role at Great Western Hospital, Wiltshire:

Job Title: Hospital Social Worker – In-Reach (Great Western Hospital)
Location: Great Western Hospital, Wiltshire
Job Type: Full-Time
Pay Rate: £36.54 per hour LTD or 27.92 PAYE
Working Pattern: Monday to Friday, on-site daily (no remote working)
About the Role Wiltshire Council is seeking a dedicated and experienced Hospital Social Worker to join our In-Reach Team based at Great Western Hospital (GWH). This is a full-time, high-impact role with a clear focus on facilitating safe and timely hospital discharges.
There is no case holding required. Instead, your core responsibility will be to work closely with health colleagues to support customers who do not require a formal hospital admission, enabling them to return home safely with an appropriate package of care.
Key Responsibilities
  • Work collaboratively with NHS and community teams to support early discharge planning.
  • Assess needs and coordinate timely support packages for customers ready to return home.
  • Ensure decisions are person-centred and promote independence.
  • Maintain accurate and timely documentation in accordance with policies and procedures.
What We're Looking For Essential:
  • Qualified Social Worker with current Social Work England registration.
  • Strong experience in hospital discharge or adult social care settings.
  • Ability to work effectively within a multi-disciplinary team.
  • Confident decision-making and problem-solving skills under pressure.
Desirable:
  • Knowledge of discharge to assess (D2A) models and relevant legislation (e.g., Care Act 2014).
  • Experience in fast-paced hospital or health settings.
