The position involves working with adults aged over 18 who require support to enable safe and timely transfer of care from acute hospital settings.
The role will involve working with individuals who are older, have diagnoses of dementia, physical disabilities, mental health conditions, and mild to moderate learning disabilities and/or autism.
The team operates within the 3 Conversation model, focusing on strength-based practice.
It works within the frameworks of the Care Act 2014 and the Mental Capacity Act 2005, ensuring a focus on adult safeguarding and meeting organisational statutory duties.
The Onward Care Team, based at Worcester Royal Hospital and the Alexandra Hospital, operates a 7-day service. It is a multi-disciplinary team comprising Worcestershire County Council and Health and Care Trust staff, emphasizing multi-agency partnership working. The team offers a supportive environment for career development.
