The JFS Aging Care Manager works with individuals 55 years of age and older and their loved ones to problem solve and guide them on their paths to safety, security, wellness, fulfillment, and future planning. Together, the Aging Care Manager, the client and family develop and implement a personalized care plan that meets the individual’s needs to support their quality of life. The JFS Aging Care Manager will connect individuals to programs that help the older adult stay independent, connected, and engaged as they age. The JFS Aging Care Manger will also engage with holocaust survivors and their families to ensure that they are connected to services, cultural activities, and benefits that they are entitled to.

Salary & Benefits:
This part-time offers an hourly rate between $21.00 - $24.20/hour. You'll be eligible to participate in our health and dental insurance plans, with JFS contributing a prorated amount annually.
