Locum Social worker – Intermediate Care Team – Southwark, earning £32 per hour for an initial 3-month period, full time. This is an exciting opportunity to join a dynamic team in Southwark, where you will use your expertise to make meaningful contributions within the community. You will be working with the Intermediate Care Team in a role focused on hospital admission avoidance, providing urgent response services. You will be embedded in a collaborative multi-disciplinary team alongside various health professionals. With the role being fast-paced, you will manage a caseload of 5-7 cases, typically resolving them within 10 days.

What you will do:
  • Participate in the Southwark @Home/Urgent pathway to support hospital admission avoidance.
  • Manage a caseload of 5-7 cases, typically moving cases on within 10 days.
  • Implement bridging urgent care interventions and collaborate within the MDT to stabilise residents.
  • Transfer cases to Reablement and complete Care Act Assessments.
  • Develop Care and Support Plans and conduct reviews of existing care.
  • Address safeguarding concerns and engage in Mental Capacity and Best Interests Decision Making.
Location and transport:

The team is based at the Main Adult Social Care building, 132 Queens Road, London, SE15 2HP, conveniently located near Queens Road Peckham Station with overground and mainline train access. Although primarily office-based for duty days, there’s flexibility to work from home on non-duty days, maintaining a balanced working style.

Experience required:
  • Essential: Social Worker qualified and registered, with a minimum of 2 years post-qualifying experience.
  • Desirable: Experience in fast-paced environments such as Urgent Response or hospital discharge.
  • Desirable: Understanding of Reablement ethos and strengths-based practice.
