Job summary

Liquid Personnel is recruiting an Adult's Social Worker to join its Integrated Discharge team in Nottingham. The role supports adults with complex care needs to achieve safe, timely discharges from hospital.

You will work in a multidisciplinary environment, undertaking Care Act assessments, coordinating services, managing risk, and developing person-centred discharge plans to promote independence and wellbeing.

Qualifications

  • Professional qualification in social work and registration with Social Work England.
  • significantly post-qualified experience within Adult Social Care, hospital discharge services, or a related setting.
  • Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.

Responsibilities

  • Supporting safe, timely, and effective hospital discharge planning for adults with complex health and social care needs.
  • Undertaking Care Act 2014 assessments, reviews, and reassessments to identify eligible needs and facilitate appropriate discharge arrangements.
  • Developing and implementing person-centred Care and Support Plans that promote independence, recovery, and positive outcomes.
  • Working collaboratively with hospital teams, health professionals, community services, carers, families, and partner agencies to coordinate discharge pathways.
  • Identifying opportunities for reablement, rehabilitation, and preventative support to reduce long-term dependency on services.
  • Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Managing safeguarding concerns and undertaking safeguarding interventions where required to ensure adults are protected from abuse, neglect, and exploitation.
  • Assessing and managing risk, ensuring discharge plans appropriately balance safety, independence, and individual choice.
  • Coordinating community-based services, equipment, care packages, and support arrangements to facilitate successful discharge outcomes.
  • Supporting adults and carers through periods of change, providing advice, guidance, advocacy, and practical support.
  • Participating in multidisciplinary meetings, discharge planning discussions, safeguarding meetings, and case reviews.
  • Maintaining accurate, timely, and high-quality case records, assessments, reports, and support plans in accordance with legislative and organisational requirements.

Job description

Liquid Personnel is recruiting an Adult's Social Worker to join its client’s Integrated Discharge team, based in Nottingham. This is an excellent opportunity for a skilled practitioner to work within a fast-paced hospital discharge service, supporting adults with complex care and support needs to achieve safe, timely, and effective discharges from hospital.


The successful candidate will work as part of a multidisciplinary team alongside health professionals, community services, carers, families, and partner agencies to promote independence, wellbeing, and positive outcomes. Using strengths-based and person-centred approaches, you will undertake statutory assessments, coordinate support services, manage risk, and develop discharge plans that enable individuals to return safely to their communities with the appropriate support in place.


This role requires a proactive Social Worker who can work effectively under pressure, manage competing priorities, and support individuals through periods of transition while ensuring compliance with Adult Social Care legislation and best practice.


What will your responsibilities be?

Qualifications and Experience:

To be successful in this role you must have;



  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.

  • Registration: Must be registered with Social Work England.

  • Experience: Significant post-qualified experience within Adult Social Care, hospital discharge services, or a related setting.

  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.

  • Hospital Discharge Experience: Experience supporting adults through hospital discharge pathways and coordinating community-based support arrangements.

  • Assessment Skills: Proven ability to undertake complex assessments, reviews, support planning, and risk management interventions.

  • Safeguarding Experience: Experience identifying and responding to safeguarding concerns involving vulnerable adults.

  • Multi-Agency Working: Strong experience working collaboratively with NHS services, community teams, carers, and partner organisations.

  • Communication Skills: Excellent communication, report-writing, negotiation, and decision-making skills.

  • Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.

  • Driving Licence: Full UK driving licence and access to a vehicle is desirable.
