Enfield Council is recruiting a Social Worker to join the Assessment and Prevention Hub in the Older People and Physical Disabilities Service. You will promote independence, support recovery, and improve outcomes for Enfield residents.

As a trusted assessor, you will complete strengths-based assessments, develop tailored support plans, and work with health services and families to deliver timely, person-centred care in the community and post-discharge settings.

Qualifications

• Registered Social Worker with Social Work England.
• Sound knowledge of the Care Act 2014 and safeguarding processes.
• Ability to complete strengths-based, needs-led assessments and support planning.
• Commitment to multidisciplinary working and partnership approaches.
• Willingness to undertake Trusted Assessor duties and assess for minor aids and adaptations.

Responsabilités

• Carry out timely strengths-based assessments, reviews and short-term case management.
• Develop person-centred support plans to ensure appropriate care levels.
• Support adults after hospital discharge, illness, rehabilitation, recuperation, or health decline at home.
• Complete assessments and support planning in community settings, step-down beds, flexi flats, residential placements and post-discharge pathways.
• Collaborate with health professionals, community partners and families to achieve positive outcomes.
