All About You
- You have current registration with Social Work England
- You have an in depth knowledge of current legislation and guidance, as well as current national policy and practice developments
- You have a full Drivers Licence and the ability to travel between different locations
- You are willing to learn new things and able to maintain your own continuing professional development.

Our team provides essential support for adult service users, ensuring safe and timely discharges from hospital. Working closely with Peterborough City Hospitals multi-disciplinary teams, you will enable discharges within the Discharge to Assess framework.
