An exciting opportunity for a Social Worker has arisen in the Warwick Hospital Social Care Team to support effective discharges and assessments. Ideal candidates will possess a Social Work qualification, a clean driver's license, and at least 12 months of post-qualifying experience. Applicants should be adept in decision-making and working collaboratively in a dynamic team. The role requires flexibility, including occasional weekend work, to ensure timely support and care delivery for diverse service user groups.
About the Role and Team
An exciting and rewarding opportunity has arisen for the recruitment of a Social Workerin theWarwick Hospital Social Care Team (HSCT). The post holder should have a minimum of 12 months post qualifying experience and have successfully completed ASYE.

We have three hospital sites across Warwickshire County Council. The Warwick hospital social care team support the discharge of customers from hospitals across South Warwickshire.
We are a dedicated and supportiveteam. We aim to respond quickly and efficiently by offering strengths-based assessments and providing support to customers and their carers within a fast-paced environment. As well as supporting with discharges the team also follow through with assessments and reviews in the community and utilise Discharge to Assess (D2A) pathways.
The teams have a mixture of staff skill sets and roles that include:

Admin
Age UK social prescribers
Trusted Assessors
The team provide a high-quality professional assessment, support planning and review service to customers. We support customers to be discharged from hospital to residential care homes for a period of assessment (D2A) or to return home with the support they require. Practitioners also work with customers who require a safeguarding response or are at risk due to self-neglect. As well as working with the Care Act, practitioners complete a significant number of Mental Capacity assessments around key decisions in the hospital discharge pathway.
We work closely with health colleagues on the wards as well as the Trust’s Integrated Discharge Team offering ample multidisciplinary experience. The team work with a mix of office based and remote working.
We are a friendly and supportive team. You will be provided with monthly supervision alongside monthly team meetings and separate 4 weekly team CPD sessions.
The postholder will carry a caseload of hospital discharge cases and will be required to work with all service user groups to meet service demands. Your normal place of work will be Warwick Acute Hospital but in the course of your duties you will be required to travel and visit locations within the South of the county. However; you may also be required from time to time to visit and work at other locations, for such times as we may reasonably determine as necessary for the proper performance of your duties.
