• Minimum of 2 years post-qualified experience in social work.

• Rate of £28.40 per hour will be paid via an umbrella company and is inclusive of HPIL; this rate is firmly set.

• Complete comprehensive assessments and care plans to aid the transition of individuals from temporary care facilities to their long-term providers.
• Work two days a week in the office, with the flexibility to conduct home visits as part of the support network.
• Participate actively in the duty rota, ensuring that colleagues and service users receive timely help and interventions.
• Engage with multidisciplinary teams to deliver holistic care and support plans, collaborating with healthcare professionals for the best client outcomes.
