Our Adult Social Care service is composed of highly professional and supportive multidisciplinary teams. We have a “can do” and “no wrong door” ethos. Our social care teams work together as one service and go the extra mile to ensure our residents have the right support.

We currently have vacancies for experienced and motivated Social Workers within the Adult Social Care Hospital Team who can respond to general enquiries and crisis situations.

We are seeking candidates that are able to provide seven day service coverage, including weekends, based at acute hospitals on an adhoc basis as required.

About you…

You will be a qualified Social Worker registered with Social Work England, with at least two years of experience working in statutory social services, typically within a local authority setting.

You will bring excellent knowledge of best practice and legislation, and approach your work with professionalism, motivation, and a “can do” attitude.

You will be committed to working in a strength-based, person-centred way to support people in achieving better outcomes and greater independence.

Our Adult Social Care Hospital Team plays a vital role in supporting some of our most vulnerable residents to get their lives firmly back on track. We use a strength-based approach, placing the person at the heart of everything we do.

Our social workers are given the time to build meaningful relationships and understand the individual needs of those they support—because we believe in working with people to change lives for the better.

We work closely with health and community partners to ensure that people only go into hospital when absolutely necessary—and when they do, we help them return home as quickly and safely as possible. Our intermediate care and discharge-to-assess services are designed to keep people in their own homes, where they feel most comfortable and supported.

Through regular multidisciplinary forums, our residents benefit from coordinated, holistic care that is responsive and seamless. We understand the pressures that can come with working in Adult Social Care, which is why we are deeply committed to supporting our staff. You will receive regular supervision, access to training and opportunities to grow and develop in your role. We actively listen to our team, encourage feedback, and value suggestions that help us improve the way we deliver services together.

Key responsibilities
  • Manage a caseload, organising and prioritising the workload effectively, with regular supervision and guidance.
  • Work as part of a multi-disciplinary team, connecting people and their families to their community and social networks enabling them to live within their own communities, and to ensure people have the right care, at the right time in the right place.
  • Take a strengths-based approach to support people to live the best life they can and reduce dependence on long term care.
  • Support in delivering and maintaining high standards of performance across the service
  • Work within allocated resources as set out in our scheme of delegation.
  • Take an evidence-based approach to practice.
  • Take an active part in your own supervision and appraisal in accordance with the departmental supervision policy and council's appraisal policy.
  • Take responsibility for your own learning, including mandatory and recommended training,
  • Lead on specific areas of work as required.
  • 28 days* increasing to 33 days* after 3 years’ continuous service.
