Job summary

London Borough of Sutton is seeking a Social Worker to join the Hospital Pathways Team within Adult Social Care. You will conduct assessments, manage complex cases, and craft high-quality care plans to support adults in regaining independence and safe discharge from hospital.

The role requires current registration with Social Work England, 2 years PQE and strong communication. A welcoming, supportive team environment and ongoing professional development are offered.

Qualifications

• Qualified Social Worker with current registration with Social Work England and eligibility to work in the UK.
• 2 years post qualifying experience working in adult services.
• Experience in assessment, care planning and discharge planning.

Responsibilities

• Conduct assessments and manage complex cases within Hospital Pathways Team.
• Develop high-standard person-centred care plans in collaboration with professionals and families.
• Coordinate discharge planning and liaise with external agencies.
