Job Title: Social Worker - Hospital Discharge - Cheshire East
Location: Cheshire East (Hybrid)
Pay Rate: £35 per hour

About the Role:

Cheshire East (Macclesfield) Council is seeking a dedicated Social Worker to join their dynamic Hospital Discharge Team. This role offers the opportunity to make a meaningful impact on patients' lives by ensuring smooth transitions from hospital to home.

What will your responsibilities be?

In this role, you will assess the needs of individuals being discharged from hospital to develop tailored care plans that promote independence and well-being. You will collaborate with hospital staff, families, and external providers to coordinate effective discharge planning and support packages. Conducting risk assessments and adhering to safeguarding protocols to protect vulnerable adults will be essential. You will implement the Care Act, Mental Capacity Act, and other relevant legislation to ensure compliance with statutory responsibilities. Additionally, you will facilitate access to community resources and services, supporting individuals to reintegrate into their home or alternative care settings. Maintaining clear, accurate, and timely records, adhering to council policies and best practice guidelines, and engaging in reflective supervision and contributing to service improvement initiatives will be key aspects of your role.

Essential Requirements:

  • Social Work England registration
