Job Description:

Are you looking for the opportunity to join a progressive hospital discharge Social Work team? As a result of a new project to provide a Social Work presence in our acute hospitals we are looking for experienced Social Workers to join us.

About the role

Our Hospital Discharge Assessment Team (HDAT) provides adult social care support, in line with the Care Act 2014 and with regard to the Mental Capacity Act 2005, to residents of North Somerset who have ongoing social care needs and are:

  1. Preparing to leave hospital after an in-patient stay,
  2. Preparing to leave the community-based Discharge to Assess (D2A) service following a recent hospital discharge,
  3. Preparing to leave the Reablement Service following a recent hospital and D2A discharge.

The team consists of Senior Social Workers, Social Workers, Occupational Therapists, and Adult Social Care Workers. Staff undertake Care Act assessments, mental capacity and best interest assessments. Staff identify eligible needs and produce care and support plans to ensure a person’s needs are met effectively for the longer term. Staff advocate for the individual whilst working positively with family members and carers, together with other professionals involved with the person, for the best outcomes.

Staff work proactively in a strengths-based way to ensure people are also able to draw on their own abilities and support networks for a quality of life, in addition to care provision. Staff work hard in a fast-paced environment to support timely discharge with minimal days lost.

Our team follows the flexible worker model, which means that desk time is shared between working from home and in our offices in Clevedon. Most of the assessment work is undertaken in the community, in care homes or in people’s own homes. You will also be part of a rota to provide a Social Work presence in the acute hospitals that serve our area, to support the “Home First” model in the Transfer of Care Hubs to facilitate effective discharges, and to assess where needs are particularly complex and unable to be supported through the usual health D2A routes. Staff are using skills, knowledge and expertise to help system partners explore the appropriate and timely options; to enable people to return to a maximised quality of life following the disruption of an acute ill health episode and a hospital admission.

This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.

Valid driving licence and own transport, or being able to provide an alternative, suitable method of travel is an essential condition for this role.

This position is currently based at our Castlewood offices in Clevedon. Please be advised that this office will be closed by 2025. Consultation is currently underway to determine the new location for colleagues based in Castlewood; This will be at Weston-super-Mare or within North Somerset.
