Our Social Workers must be able to listen hard to the people of Gloucestershire, to understand what really matters and to connect them to resources that help them continue with their chosen life, either with support or independently. You will also be working with individuals to help them to regain control of their lives in a crisis. We use the Three Conversation approach, a model that re-emphasises focus on people, their strengths and the networks and community around them.
What can we offer you?
In recognition of the vital contribution you make, we offer:
* £35 per hour.
* £150 Accommodation / mileage weekly allowance (T&Cs attached) - receipts are required
* Capped caseloads at 25.
* Guaranteed minimum 3-month placement (subject to renewal)
* Dedicated support with regular supervision and guidance from line management.
Key Responsibilities:
* Conducting comprehensive assessments of patients' social care needs in collaboration with multidisciplinary teams, including medical professionals, nurses, and occupational therapists.
* Facilitating effective communication and coordination between hospital staff, community-based services, and other relevant agencies to ensure seamless transitions for patients from hospital to home or alternative care settings.
* Providing support and guidance to patients and their families throughout the discharge planning process, addressing any concerns or challenges they may encounter.
* Collaborating with local authorities and partner organizations to arrange appropriate care packages and support services tailored to patients' individual needs and preferences.
* Advocating for patients' rights and preferences, ensuring their voices are heard and their choices are respected in the planning and delivery of care.
* Maintaining accurate and up-to-date documentation in accordance with regulatory standards and organizational policies.
* Participating in team meetings, case conferences, and training sessions to share knowledge and best practices, as well as contributing to the continuous improvement of service delivery.
Requirements:
* Bachelors degree in social work or related field.
* Registration with the Social Work England (SWE)
