Charles Hunter Associates is seeking a Social Worker to join the Hospital Discharge Team. The role requires a Social Work qualification and a minimum of 2 years post-qualified experience.

You will assess patients for safe discharge, coordinating care and ensuring needs are met to enable living as independently as possible. The ideal candidate will manage time effectively, possess strong interpersonal skills, and engage with diverse service users.

Formación

• Social Work degree (Degree/DipSW/CQSW) with a minimum of 2 years' experience.
• Experience working with adults is essential to succeed in this role.
• Ability to thrive in a fast-paced environment and engage with service users from diverse backgrounds.

Responsabilidades

• Assess patients to determine discharge needs and safety.
• Prioritise workload and manage time effectively for timely discharges.
• Engage with ward staff and work from home when appropriate to support patient destinations.

We are looking for a Social Worker to join our Hospital Discharge Team.

About the team

The team are responsible for the safe discharge of adult patients from Hospital,
You will be responsible for the assessment of each patient ensuring that all their needs are met and that they can continue to live their lives as freely as possible.this position involves assessments from home as well as being present on the wards when necessary.

About you

The ideal Social Worker will be able to manage time effectively to prioritise their workload as well as have good interpersonal skills.Experience working with adults, the ability to work in a fast-paced environment and the ability to be able to engage with service users from diverse backgrounds is key to the success of the role.A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 2 years' experience is essential in order to be considered for this role.
