We promote the home first model and discharge to assess approach and believe with the appropriate safe level of care clients can maintain their independence and stay at home, safely where possible. We work closely with our reablement teams and our inpatient rehabilitation providers to maximise client’s independence and build confidence and positive outcomes for the client we work with. Where necessary we work with our local Integrated Commissioning Boards (ICB’s) to ensure people we serve get access to continuing care funding.
