What you will do:

  1. Undertake statutory reviews in alignment with the Care Act 2014 and other related legislation.
  2. Conduct comprehensive assessments of patient needs, risks, and necessary support plans.
  3. Play an integral role in multi-disciplinary team meetings to ensure person-centred care planning and effective discharge planning.
  4. Collaborate closely with NHS colleagues, care providers, and families to develop discharge support strategies that promote autonomy and safety.
  5. Monitor and review care arrangements regularly, ensuring they stay suitable and effective.
  6. Maintain detailed, current case records using internal electronic systems while adhering to data protection protocols.
  7. Identify and address any safeguarding concerns, following established safeguarding policies and procedures.
  8. Offer advocacy, support, and guidance to patients and their families throughout the discharge process.

Minimum Requirements:

  1. Qualified Social Worker with registration with Social Work England.
  2. Proven experience in a hospital or health and social care setting.
  3. Thorough understanding of the Care Act 2014, Mental Capacity Act, and safeguarding procedures.
  4. Ability to handle a complex caseload and make informed, timely decisions.
  5. Exceptional communication, interpersonal, and report-writing skills.
  6. Proficiency in using electronic case management systems.
