What you will do

• Deliver outreach and in-reach interventions, aimed at supporting individuals with complex needs including mental health issues.
• Collaborate with local homelessness services, housing providers, and other agencies to implement support strategies.
• Provide tailored support plans to individuals to help them sustain housing and work towards long-term stability.
• Address unmanaged behaviours such as hoarding or aggression through non-statutory commissioned services.
• Participate actively in the Advice and Wellbeing Hub team, ensuring cohesive service delivery.
