What You�ll Do:

As a key member of our interdisciplinary care team, you�ll support patients and families by providing emotional guidance, resource coordination, and advocacy in the home health and hospice settings. You�ll collaborate closely with nurses, therapists, and physicians to ensure comprehensive care and a strong support system for every patient.

Your Day-to-Day Might Include:

� Offering emotional and psychosocial support to patients and families
� Helping patients and caregivers set healthy boundaries and goals
� Identifying high-risk situations and coordinating timely interventions
� Referring families to community services and financial assistance programs
� Supporting hospice patients with bereavement and respite care planning
� Participating in team case conferences and care planning
� Providing supervision to other social work staff (if licensed at LICSW level)
� Documenting assessments, plans, and outcomes in a timely and accurate manner

� Master�s Degree in Social Work (MSW) preferred
