Job summary

Jobtailor is recruiting a Qualified Social Worker in East Sussex to cover Lewes and surrounding areas. You will work with Adult Social Care’s Out of County Hospital Team, supporting safe discharge from hospital to people’s homes and promoting the home first model.

Travel across East Sussex, including rural areas, and collaborate with hospital trusts, reablement teams and Integrated Commissioning Boards to help people access continuing care funding.

Qualifications

• Qualified Social Worker
• Registered Social Worker with Social Work England
• Social work qualification
• Enhanced DBS check
• Evidence of an overseas police check if lived or worked abroad
• Applicants requiring UK work sponsorship must disclose this so eligibility can be checked

Responsibilities

• Work within Adult Social Care’s Out of County Hospital Team covering Lewes and North Wealden.
• Occasionally undertake work for the Community Hospital Social Work Team covering Eastbourne and Hastings
• Collaborate with local partners and hospital trusts to support safe discharge from hospital to people’s homes
• Promote the home first model and discharge to assess approach
• Support clients to maintain independence and remain safely at home where possible
• Work with reablement teams and inpatient rehabilitation providers to maximise client independence and build confidence
• Work with local Integrated Commissioning Boards to help people access continuing care funding
• Travel across East Sussex, including rural areas
• Participate in interview panel process if applicable
