The role:
The Front Door team is where people begin their journey with Adult Social Care. It is the place where we listen first, understand what matters, and work alongside residents to find the right support at the right time. We work with adults across the borough who may be experiencing changes in their health, independence, or wellbeing. This includes people living with physical or sensory impairments, age-related conditions, mental health needs, neurodivergence, or substance use challenges. Our role is to offer early, short-term support, helping people regain confidence, maintain independence, and stay connected to their communities. Most of the time, we work with people for around six to eight weeks, supporting them to move forward - either independently or with ongoing support from community or specialist services where needed.
