Social Worker - Fostering Retention

We are seeking a Social Worker to support young people and adults transitioning from children's services into adult care. This vital role ensures individuals experience a smooth transition, promoting independence, stability, and positive outcomes.

Key Responsibilities:

  1. Conduct comprehensive assessments and develop tailored transition plans.
  2. Provide guidance and advocacy to young people and their families throughout the transition.
  3. Collaborate with multidisciplinary teams, including education, health, and social care professionals.
  4. Ensure compliance with safeguarding policies and relevant legislation.
  5. Maintain accurate records and prepare detailed reports to support decision-making.
  6. Promote independence, inclusion, and person-centered care approaches.
