Cheshire East Council is currently involved in some innovative work streams within the Community Teams as they are part of the wider Care Community Partnerships. This is an exciting opportunity for a Social Worker to increase their core skills and Social Work values. We are looking for a Social Worker who can work in a fast-paced adult community setting and will need to have good communication skills, decision making skills and the ability to be creative in their work. The successful applicant will need to be able to adopt a flexible approach to their work and be a good team player. The successful candidates will support the Adult Community teams by supporting with hospital prevention, hospital discharges, assessments, safeguarding concerns, capacity and best interest decisions and have a good understanding of risks.
The Candidates will need to have an understanding of person-centred care and focused on strengths-based practice, either within the individual’s own home, hospital, or Care Home. You also need to have an understanding of Mental Capacity, Best Interests, and the adult safeguarding process. Cheshire East Council have great opportunities for learning and development, and you will be part of supportive and well managed team, and some home working is available, but you will be provided with management support and oversite as needed.
What we are looking for:
It will be essential for the post holder to work autonomously within the Council’s policies and professional practice, in addition to maintaining accurate and comprehensive client documentation.
The post holder will be based with the Macclesfield Adult Community team. However, it may be a necessary to travel to other areas within the Council. Therefore, the candidate needs to be able to drive and have access to a vehicle to access the community and undertake visits and training needs
