Join a dynamic Adult Community team dedicated to providing support and intervention for individuals aged 18 and above, living with physical disabilities, chronic conditions, autism, and sensory impairments.

Our teams offer support and intervention to people aged 18 plus, who live with physical disabilities, chronic and long-term conditions, autism and sensory impairment. We work in a supportive environment and are committed to professional and career development, providing monthly supervision and offering flexible working arrangements.

About the Role

As a crucial member of our team, you'll collaborate with individuals, families, and caregivers, conduct Care Act 2014 assessments, evaluate needs, and manage both short-term and long-term cases. Dive into advancing safeguarding inquiries, handling Court of Protection matters, and maintaining close collaboration with healthcare and multidisciplinary professionals.
