Social Worker - Discharge to Assess Hospital Team

Main duties of the job

About the Role:

  • Complete timely Care Act assessments for higher intensity case management with a strength-based focus, delivering positive outcomes for individuals in Reading.
  • Conduct post-discharge reviews to ensure care plans are accurate and individuals' needs are met.
  • Promote the use of direct payments to provide individuals with flexibility in meeting their needs.
  • Encourage a reablement approach for all individuals referred to adult social care.
  • Engage with families and carers, ensuring carers assessments are completed to support those with caring roles.
  • Develop effective relationships with providers, voluntary services, and health professionals.
  • Supervise staff to promote professional development and provide support.
  • Contribute to team efficiency and deputise for senior staff when needed.
  • Work as part of a high-performing, outcome-focused team that embodies the principles of TEAM Reading.
