Job summary

Northamptonshire Children's Trust is seeking an experienced Social Worker to join the Children with Disabilities Team in Kettering. You will manage a varied caseload across CIN, CP and Children in Care, delivering assessments, care planning and timely reviews to promote positive outcomes for children and families.

Working within a multi-disciplinary environment, you will collaborate with education and health partners, maintain robust case records, and ensure safeguarding and voice of the child

Qualifications

• Social Work England registration.
• Experience within a Children with Disabilities Team.
• Experience managing CIN, CP, and CIC cases.
• Strong assessment, care planning and safeguarding skills.
• Experience in multi-disciplinary, multi-agency environments.
• Excellent communication, organisation and case management abilities.
• Child-centred approach with commitment to positive outcomes.

Responsibilities

• Manage cases involving CIN, CP, and CIC.
• Undertake comprehensive assessments of children and families.
• Assess needs and develop effective care and support plans.
• Arrange, review and monitor care packages and short breaks.
• Complete statutory visits, assessments, reports and reviews within timescales.
• Collaborate with education, health professionals and partner agencies.
• Contribute to safeguarding and risk management.
• Ensure children's voices remain central to interventions.
• Maintain accurate case recordings and documentation.
• Attend multi-agency meetings, reviews and planning discussions.
