A leading company in the social work sector is seeking a professional with a Bachelor's Degree in Social Work for a role focused on referral management and case oversight. Responsibilities include overseeing referrals, monitoring capacities, and managing casework for vulnerable individuals. Ideal candidates should have relevant experience, a commitment to supporting the destitute, and the ability to start immediately.
Working Location:Caldecott
Contract Duration: 1 year
Working Hours: Monday – Thursday: 8.30am – 6pm, Friday: 8.30am – 5.30pm
Basic Salary: $3,160 per month
Responsibilities:
Referral Management:
· Oversee the referral process from initiation to resolution, ensuring timely and efficient handling of all referrals.
· Coordinate with internal teams and external partners to facilitate smooth referral pathways.
· Monitor and ensure the proper documentation and follow-up of referrals to ensure no case is overlooked.
· Streamline, review and refresh to keep referral process relevant.
Monitoring Capacity and Vacancies
· Engage partners and stakeholders in supporting rough sleepers and vulnerable individuals.
· Monitor the capacity and vacancies across S3Ps, TSes and Welfare Homes (WHs).
· Work with Data Team to Design and Develop systems interface to provide updated data on vacancies for smoother placement.
Oversight of Case Management of Residents in Shelters and Institutional Care
· Maintain oversight of the rehabilitation and reintegration of the residents into society.
· Review cases across shelters and Institutional care to ensure that residents are rightly-sited.
· Review cases for extensions of stay.
· Centrally monitor and manage the repository of data collected on residential care needs.
Secretariat and Staffing Work
· Perform secretariate work for the relevant Review Committee and other necessary platforms.
· Plan, schedule and coordinate the meetings with various stakeholders.
Requirement:
· Bachelor’s Degree in Social Work or other relevant disciplines.
· Relevant experience with casework and case management.
· Interest in working with the poor, homeless and destitute.
· Immediate Starters are preferred (Not serving any notice period)
