Sirona Medical Social Work department is currently seeking a qualified and motivated Social Worker to join the Community Neuro Rehabilitation Team (CNRT) and the Neuro:rehab Enhanced Transition Team (NETT) on an agency basis.

Position Overview

We are currently looking for a qualified and motivated Social Worker to join our agency at Sirona Medical Social Work department. This is an exciting opportunity to work with multidisciplinary teams, including Occupational Therapists, Physiotherapists, Speech and Language Therapists, Neuropsychologists, Nurses, Doctors, and rehab support workers.

Your Role
  • Collaborate with multidisciplinary teams, contributing your social care expertise to holistic patient plans and attending weekly in-person MDM meetings for both NETT and CNRT, in addition to online huddles.
  • Provide crucial adult social care interventions, including Care Act Assessments, MCA, Safeguarding, advice, information, and referrals, focusing on individual patient goals and service targets to ensure efficient patient flow and hospital admission avoidance.
  • Support the NETT team in facilitating smooth transitions for complex neurological patients from hospital/rehab units back into the community. Your short-term, intensive support (up to 12 weeks) will help reduce hospital stays and prevent readmissions.
  • Within CNRT, work with individuals over 18 with neurological conditions, providing support for recovery, managing difficulties, and promoting wellbeing. An understanding of progressive conditions and ongoing neuro disability management is essential.
  • Manage your caseload with a degree of autonomy, using your professional judgment while benefiting from guidance from your Deputy Team Manager.
  • Empower patients to achieve their goals and maximize their rehabilitation potential.
  • Deliver adult social care interventions effectively, with a strong understanding of neurorehabilitation principles, and work efficiently in a fast-paced environment focusing on short-term interventions and outcomes.
  • Exhibit excellent communication and interpersonal skills to collaborate within multidisciplinary teams and manage your workload autonomously.
Qualifications
  • BA Honors or Master's Degree in Social Work
  • Social Work England Registration
  • Eligibility to work in the UK
  • Full UK driver's license and access to a vehicle (if applicable)
  • At least three years of post-qualifying experience employed directly within an LA children's service in England
  • Strong interpersonal and communication skills for a fast-paced healthcare environment
  • Empathetic, patient-centered approach with a dedication to high-quality care
  • Ability to work collaboratively within a multidisciplinary team
