Our Client Lewisham Council is looking for a Qualified Social Worker to join their Community Mental Health Team.
The CMHT offers support and a range of interventions for adults of working age who have serious long-term mental health conditions. The social worker will act as a care coordinator alongside colleagues from South London and Maudsley NHS Foundation and the voluntary sector. They will hold a caseload of people on CPA, as well as covering the team duty rota and leading on adult safeguarding and Care Act assessments within the multi-disciplinary team.
