Pay Rate: £30 per hour
Job Description:Liquid Personnel is recruiting Social Worker for it client's Community Mental Health Team (CMHT) located at Rochdale.
What will your responsibilities be?- Undertake Strengths-Based Needs Assessments and review care packages for adults with severe and enduring mental health needs
- Update care plans and complete Mental Capacity Assessments where required
- Carry out safeguarding work in line with statutory duties when appropriate
- Support timely hospital discharges for individuals under secondary mental health services
- Work with both in-borough and out-of-borough placements; understanding of mental health pathways is beneficial
