Job Title: Social Worker – Referral & Assessment Team

Location: Wokingham

Rate: £38 per hour

Overview:

An exciting opportunity has arisen at Wokingham for a highly skilled Locum Social Worker to join the Referral & Assessment Team. The team works closely with families and professionals to identify and provide appropriate support, ensuring safe and effective parenting. We value relationship-based practice to ensure meaningful and professional interactions with children and families.

Responsibilities:
  • Work within a supportive Multi-Disciplinary Team (MDT), planning, monitoring, and evaluating cases.
  • Follow Social Work England standards, professional values, and code of conduct.
  • Conduct needs-led assessments, care planning, reviews, and multidisciplinary assessments.
  • Support individuals and families, involving them in decision-making, and providing guidance and referrals.
  • Practice according to the Professional Capabilities Framework (PCF) and maintain professional standards.
