Responsibilities

• Collaborate with partners to identify what children and families need to make lasting positive change and support permanence options.
• Advocate for children and young people, ensuring their voices are heard in decision making.
• Undertake comprehensive assessments and develop plans led by families’ strengths, needs and aspirations.
• Develop and implement care plans tailored to each child’s needs.
• Monitor and review progress of children’s plans and adjust care plans as needed.
• Provide emotional and practical support to children to help them navigate challenges and build resilience.
• Work to achieve permanency and outstanding outcomes through relationship-based practice.
