Social Worker – Cared for Team

We are looking for a Social Worker to support young people and adults as they move from children’s services into adult care. This is a crucial role in ensuring individuals experience a smooth and well-supported transition, enabling them to achieve independence, stability, and the best possible outcomes.

About you.

  • Hold a social work degree.
  • Right to work in the UK and hold clean UK driving license.
  • Registered with SWE
  • Post qualified experience within children and family services.

Key Responsibilities:

  • Conduct comprehensive assessments and create tailored transition plans.
  • Provide guidance and advocacy to young people and their families throughout the process.
  • Collaborate with multidisciplinary teams, including education, health, and social care professionals.
  • Ensure compliance with safeguarding policies and care legislation.
  • Maintain accurate records and provide detailed reports to inform decision-making.
  • Promote independence, inclusion, and person-centered care approaches.
