Job summary

• Collaborate with psychiatrists, social workers, nurses and emergency services to ensure comprehensive patient care.
• Assess social circumstances, risks, support networks and care needs for thorough and empathetic support.
• Safeguard the rights, dignity and welfare of individuals experiencing mental health crises.
• Complete accurate statutory records, reports and legal documentation to ensure compliance.
• Support discharge planning and promote appropriate community-based care arrangements under the Mental Health Act and Mental Capacity Act.
