A leading consultancy is seeking a committed Social Worker to join their All Age Disability Service, supporting children and young people with disabilities. This role involves statutory assessments, developing care plans, and collaborating with various sectors to ensure comprehensive support for families. The ideal candidate will have significant experience, strong knowledge of relevant legislation, and a passion for improving lives.
Are you passionate about making a real impact in the lives of children and young adults with disabilities? We are looking for a committed and experienced Social Worker to join our dynamic All Age Disability Service, supporting children and young people aged 0-25 and their families.
The Role:
You'll work within a supportive multi-disciplinary team to carry out statutory assessments and deliver high-quality care and support planning under the Care Act 2014 and Children Act 1989. This includes working with children in need, child protection plans, and transitions into adulthood.
Key Responsibilities:
Undertake complex casework and statutory assessments

Develop personalised care and support plans
*
Contribute to safeguarding investigations and attend child protection conferences
*
Support young people in transition to adulthood, including preparing for independence
*
Liaise with education, health, and voluntary sectors to ensure coordinated support
*
Maintain accurate case records in line with professional and legal standards
About You:
Qualified Social Worker with current Social Work England registration
*
Minimum 2 years' post-qualifying experience (experience with 0-25 or disability services preferred)
*
Strong working knowledge of Care Act 2014, Children Act 1989, and SEND code of practice
*
Skilled in building relationships with families, young people, and partner agencies
*
Resilient, reflective, and able to manage a diverse caseload
*
Enhanced DBS required
