Join our Advice and Wellbeing Hub and play a vital role in supporting individuals to access the right care and support at the right time.The Advice and Wellbeing Hub is a busy, supportive and friendly team, working with people of all ages and backgrounds and their families and carers. We use a Conversations Count strengths-based model to promote choice and control and achieve excellent outcomes.You will work with individuals who have been referred for support for up to 6 weeks, to maximise their independence in their own homes. Promoting choice and control and using a strength-based approach.
About the Role:
The role involves you undertaking work using the Conversation Counts model, working on prevention and early support. Using your social work expertise, you will conduct Care Act assessments, safeguarding investigations, and Mental Capacity assessments, as well as facilitate best interest meetings. You will also manage a varied caseload, ensuring individuals receive the right support to maintain their independence.
This role involves working with adults with diverse needs, including individuals with learning disabilities, physical disabilities, older adults, and those affected by substance or alcohol misuse. Additionally, you will provide information, advice, and support to carers, helping them navigate available resources and services.This position offers an excellent opportunity to develop your skills and experience in Adult Social Care, delivering high-quality, person-centred interventions to individuals in their own homes. You will be part of a friendly and supportive team committed to making a meaningful difference in peoples lives.
