What You Will Do:
  1. Hold and manage a complex and varied caseload, ensuring compliance with policies and legislation.
  2. Undertake assessments and develop personalised support plans in conjunction with service users, carers, and multidisciplinary teams.
  3. Prepare reports and represent the department in judicial and multi-disciplinary forums as needed.
  4. Facilitate direct interactions with service users and their families, ensuring safeguarding procedures are observed.
  5. Ensure services are provided within financial constraints and identify any gaps to inform management and improve service delivery.
  6. Maintain up-to-date and accurate client information, adhering to data protection regulations.
  7. Support colleagues and contribute to team development by sharing knowledge and best practices.
Qualifications:
  1. Dip SW, BA or MA Social Work or equivalent / or due to receive qualification within next 5 months following application.
  2. Social Work England registered or due to receive registration within the next 5 months following application.
  3. Applied knowledge of Community Care Legislation and subsequent guidance, including Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983.
  4. Experience of completing assessments and support plans and generating these through information technology.
