Richmond and Wandsworth Adult Social Care is seeking experienced Social Workers to join the Wandsworth Locality Teams. This role focuses on adults with physical disabilities and older people, delivering holistic, strength-based assessments and coordinated care planning.

You will work in partnership with residents, carers and partner organisations, visiting families and liaising with services to promote independence while minimising risk.

Qualificações

• Qualified social worker with relevant registration.
• Experience in adult services and post-ASYE experience preferred.
• Ability to work in a person-centred, holistic, strength-based manner.

Responsabilidades

• Undertaking assessments of adults’ and carers’ needs, strengths and risks to promote independence and prevent harm.
• Working in partnership with residents, carers and partner organisations.
• Collaborating with Locality Team members to deliver services across Wandsworth.
