Social Worker - Adults Locality North Team

Job Title: Locum Social Worker - Adults Locality North Team

Specialism: Adults Locality with a focus on Learning Disabilities and Autism

Contract: 3 months - Ongoing, Full Time, 37 hours Monday to Friday (09:00 AM - 05:00 PM)

Candidates must live in Wolverhampton or be within a 30-minute commute of Wolverhampton with their own vehicle.

You will utilize your skills to complete MCA and community DOLS applications.

What you will do:

• Assess the needs of individuals with learning disabilities and autism and work collaboratively to develop personalized care plans.
• Support individuals through the dynamic support register and the building of the right support to improve their quality of life.
• Collaborate with healthcare professionals to navigate and facilitate joint funding pathways for clients' care needs.
• Apply your knowledge of the Mental Health Act to ensure compliance and support of legislation through S117 aftercare obligations and inpatient reviews.
• Prepare and process MCA and community DOLS applications to ensure clients receive appropriate care in community settings.
