  • Support patient flow from psychiatric units to ease hospital discharge pressures and facilitate movement from step-down arrangements.
  • Address complex care needs for individuals over 65 to promote successful reintegration into the community.
  • Assess individual needs and contribute to support planning and co-design.
  • Ensure regular monitoring and review of support plans for continuous improvement of wellbeing and outcomes.
  • Manage your caseload independently while upholding department and council guidelines and maintaining a positive departmental reputation.
