Social Worker - Adults Hospital D2A (Discharge to Assess) Community Team - Manchester: Earn £30 Hourly, Temporary up to 3 Months

We're offering a thrilling opportunity for a qualified Social Worker to join our Adults Hospital D2A Community Team in Manchester. With a generous hourly rate of £30 and the flexibility of a temporary role lasting up to 3 months, this position is perfect for those looking to make a substantial impact in a short time. This position offers options for full-time or part-time employment, and as we operate a 7-day service, your work may include 35 hours a week with some weekends.

What you will do:

* Provide expert guidance and support to patients and families during the discharge process, offering information and resources to support informed decision-making.

* Work flexibly across various services, including Crisis response, intermediate care, and discharge teams within the hospital setting.
