Job Title: Social Worker – Adults Intermediate Care Team (Rehab Unit)
Location: Portsmouth (Rehab Unit)
Job Type: Full-Time
Pay Rate: £27.33 LTD per hour or £21.53 PAYE
Working Pattern: 4 days on-site, 1 day remote
About the Role We are looking for an experienced and motivated Social Worker to join the Adults Intermediate Care Team based in a short-stay rehabilitation unit in Portsmouth. This role is focused on short-term interventions and is ideal for someone who thrives in a fast-paced hospital or rehab setting.
You’ll be working as part of a supportive multidisciplinary team, attending daily boardrounds and MDTs, and delivering high-quality, person-centred assessments in line with the Care Act and Mental Capacity Act.
Key Responsibilities
  • Complete timely Care Act assessments, discharge planning, and MCA assessments.
  • Attend and contribute to daily MDT meetings and boardrounds.
  • Work collaboratively with health professionals to support short-term rehab goals and safe discharge plans.
  • Maintain high standards of recording and documentation, even in a fast-paced environment.
  • Participate in team meetings, reflective practice sessions, and receive regular supervision.
What We're Looking For Essential:
  • Qualified Social Worker with current Social Work England registration.
  • Experience working in hospital discharge or intermediate care settings.
  • Strong knowledge of the Care Act, Mental Capacity Act, and strengths-based approaches.
  • Ability to manage short-term, fast-moving cases efficiently while maintaining quality.
Desirable:
  • Previous experience in rehabilitation or reablement units.
  • Confident communication and partnership working within a multidisciplinary team.
